AUSTINBURG ELEMENTARY SCHOOL
KINDERGARTEN REGISTRATION Kindergarten registration will be held at Austinburg
Elementary School on , from 8:30 to 11:30
a.m., and 12:30 to 3:30 p.m.
To be eligible for kindergarten, students must be 5 years old by September 30. Children whose birthday falls after September 30 will not enter kindergarten until the following year, unless evaluated and recommended for early entrance by the school psychologist. Registration forms are available at each elementary school office. Parents are asked to pick up and complete registration forms prior to the registration day. Please tell all neighbors, friends, and relatives about kindergarten registration. PLEASE BRING YOUR CHILD, and the following information, as required by Ohio law, with you for registration: l. Birth Certificate (Hospital
or mother’s record is not legal certification.)

If you do not have a family doctor to complete the necessary immunizations, the Ashtabula County Health Department sponsors the following clinics: • Every Tuesday, 11:00 a.m.-12:00 noon, Geneva Human Resources • Every Monday, 8:00 a.m.-5:30 p.m., Health Dept., 12 W.
