Kindergarten registration will be held in the Geneva Area City School
District on the following schedule for the 2008 – 2009 school
year.

Students whose last names begin with the letters A-L
will register from 8:30 AM to 11:30 AM. Students whose last names
begin with the letters M-Z will register from 12:30-3:30PM.
Registration
packets may be picked up in advance by stopping at any of the
school offices.
To be eligible for kindergarten, a child must be five
years old by September 30th. Children who do not meet the age
requirement will not be accepted unless evaluated and recommended for
early entrance by our school psychologist.
Please bring
the following information, along with your child, to kindergarten
registration:
1. Birth Certificate (Hospital or
mother's record is NOT legal certification.)
2. Social
Security Card
3. Completed
Registration Forms
4. Written
proof of immunization:
* DPT (4 shots, 1 booster)
*Polio (3 shots, 1 booster)
* MMR - Rubella, Rubeola, Mumps (2
doses of each)
* Hepatitis B (3 doses)
*Varicella (chicken Pox) or parental verification
of natural Chicken Pox
5. Proof of Residence
6. Proof of Legal Guardianship (If the child is not
living with both natural parents, proof of leagal
guardianship must be presented.)

If you do not have a family doctor to complete the necessary
immunizations, the Ashtabula County Health Department sponsors the
following clinics:
• Tuesday, l1:00 a.m.-l2:30 p.m., Geneva Human
Resources next to Geneva Community Center
• Monday, 8:00 a.m.-5:00 p.m., Health Dept., 12 W.
