or faxed to 440-466-8547 with the following information to fill the request:

YEAR OF GRADUATION

NAME AT THE TIME OF GRADUATION

FULL DATE OF BIRTH

CONSENT TO SEND TRANSCRIPT

WHERE IT NEEDS SENT, (with email address or physical address.)

Please allow up to 5 days turnaround. All information is needed to fill the request.
